Player Forms and Paperwork
CURRENT/UPCOMING COUGAR FOOTBALL PLAYERS
Student athletes participating in the Countryside Cougars Football Program
must obtain Student Accident Insurance and complete the necessary Athletic Participation Forms available below.
Completed participation forms for varsity level players can be emailed to Coach Schmitz
Varsity Coach Eric Schmitz
Completed Participation forms for junior varsity (JV) level players can be emailed to Coach Pipkins
JV Coach Thomas Pipkins
Anybody that did not play football last year or any incoming freshman must have these completed prior to beginning any football workouts. Anybody with physicals (EL2 form) expiring must have their physicals current in order to participate.
