Student athletes participating in the Countryside Cougars Football Program

must obtain Student Accident Insurance and complete the following Athletic Participation Forms

 

These forms and coverages are mandatory for the upcoming school year

Completed participation forms can be brought to Countryside High School's Athletic Paperwork Night

Anybody that did not play football last year or any incoming freshman must have these completed prior to beginning any football workouts. Anybody with physicals (EL2 form) expiring must have their physicals current in order to participate

PARENTS

In order to volunteer at any Countryside Cougar Football events, parents will need to complete the Pinellas County Schools Volunteer Registration Form.
